Confidentiality: All of our communication becomes part of the clinical record, which is accessible to you upon request. I will keep confidential anything you say as part of our counseling relationship, with the following exceptions: (a) you direct me in writing to disclose information to someone else, (b) it is determined you are a danger to yourself or others (including child or elder abuse), (c) I am ordered by a court to disclose information, (d) If you desire to seek reimbursement from a managed care company, the disclosure of confidential information may be required for reimbursement, (e) In case of a Medical Emergency, or (f) These rights are waived if accusations of misconduct are brought. However, even under these circumstances, only essential information will be revealed, and as much as possible, you will be informed before confidentiality is broken. In the event, the client is a minor, parents or legal guardians may be included in the counseling process as appropriate. Still, measures will be taken to safeguard confidentiality, always acting in the best interest of the client.
